Family Plans

One parent/guardian with 1 kid: $150/mo adult and $75/mo for the child ($175/mo)

One parent/guardian with 2 or more kids:  $150/mo for the adult and $50/mo per child

Couple with 1 or more kids: $150/mo per adult and $50/mo per kid

* There is a 5% discount for annual payment

$150/mo with no parent/guardian member 
$75/mo with one parent/guardian member 
$50/mo as part of a family plan

***Though CDPC strongly enourages vaccinations, we do not have in house vaccinations available.  These would need to be done at a Pharmacy or the Health Department.

There is a one time $95 sign on fee per person or per family.

I do not hold persons to a contract, but there is a 4 month minimum, and a 30 day written cancellation policy.
